Easy Enrollment
You can either pick up these forms during your tour with BMA or you can
print them out ahead of time.
Barrel of Monkeys Academy Enrollment:
Go to enrollment forms and print out the #1, #3 and #4.
1. Enrollment/Admission form
2. Copy of current Immunization Shots
3. Getting to know your child form.
4. Sign the last page of the Policy & Procedure Manual
5. Signed Food Program Form to be completed after you enroll.
6. First month of Tuition is due & Enrollment Fee is due to secure your spot(s).
